Doubrava Urology accepts most insurance plans. If your insurance is not listed below, please call our office and we will be happy to verify your coverage.

Remember, if your insurance plan (HMO, POS, Managed Choice, etc.) requires a referral for a specialist visit, you must contact your primary care provider to obtain the referral before making an appointment.

Our office will file insurance claims for each billable service we provide. Patients are responsible for deductibles, co-payments, and coinsurance as stipulated by your plan. Payment is due upon check-in or we reserve the right to reschedule your visit. We accept cash, check, Visa, and Mastercard. 


Appointments

If you need to schedule an appointment with Dr. Doubrava, please contact our office and our staff will be happy to schedule an appointment. As a courtesy to our patients, we call the day before to remind them of their appointments.

If you need to cancel or reschedule your appointment, we require at least 24 hours notice if you cannot keep your appointment. Failure to do so, may result in a $45.00 charge.

Prescription Refill Requests

Patients may request medication refills by calling our office or through their pharmacy. Please allow up to 72 hours for refills to be processed, as Dr. Doubrava is not in the office every weekday.

Dr. Doubrava requires an annual visit to approve medication refills.
